I hereby certify that I am an active member in good standing of the Country Music Association of Ontario; that I have, with my own hand, personally cast my vote on the attached ballot; and that I have not accepted any consideration for the manner in which my ballot has been cast. I understand that the term "consideration" is not limited to cash payments but extends to anything of value that may be given or exchanged for my vote. I understand that, in order for my ballot to be counted, it must be accompanied by this certificate, and that if I violate the rules of election or any provision of this certificate, my vote will be declared invalid.
